We are happy that you have decided to become a patient of Nova Surgery Center. Our main priority is to provide you personal care; and we hope that our quality services, comfortable office surroundings and caring staff will make your visit a pleasant experience.

 

To ensure that your first visit to our office is an enjoyable experience, please arrive 15 minutes prior to your appointment time.

 

Please download the new patient forms  for your convenience.

 

Please bring any insurance card or method of payment for your visit. Bring a list of your current medications and, if available, medical records from your previous OB/GYN or primary care physician.

 

Prior to your appointment, please prepare a list of questions or concerns you might have and bring them to your appointment. Physicians and staff are here to listen to and answer your questions.

 

If you cannot make it to your appointment, please notify us as soon as possible so that we can make the time slot available for another patient. We will be happy to reschedule your appointment for a more convenient date and/or time.
